SOCIAL WORKER MSW

In collaboration with nurses in the Case Management practice, provides patient/family interventions to support clinical, satisfaction, financial, and compliance related outcomes. Pace cases from a psychosocial-economic perspective by assisting patients and families in adjustment to episodes of acute and chronic illness; in coping with lifestyle changes precipitated by illness or disability; in participating in restorative health programs; in carrying out discharge recommendations; and in addressing issues of terminality. Using a variety of treatment modalities, assists patients and families in the alleviation or resolution of social, financial and emotional problems related to illness, health care, and rehabilitation. This position requires coordination with multiple health care personal and community agencies.

This position is telephonic and it is located in Des Plaines, IL.

Bilingual Spanish/English is highly preferred.

Education and/or Experience

MSW required. 3 years of clinical Social Work experience required. Acute care Social Work experience preferred. State of Illinois Social Work license is required.
